Recent literature indicates that the use of silicone PSAs in the healthcare arena
continues to expand in response to specific application needs. A number of publications
describe compositions and/or constructions designed to provide specific benefits,
such as efficient drug delivery, improved wound management/healing, greater
friendliness to human skin, and resistance to biological fluids [1, 53–63]. General
trends in using adhesives for wound-care applications have expanded into silicone
adhesives. Posten [64] conducted an extensive investigation comprising 27 separate
studies and concluded that silicone gel sheeting was superior to other occlusive
dressings in the treatment and management of scars. Since then, the market has seen
a proliferation of scar therapies utilizing silicone gel sheeting. Several professional
wound dressings are currently available that use silicone gels as the skin-contact
adhesive. The need for medical dressings utilizing adhesives that can be applied
to fragile skin types without causing irritation and that can then be removed without
traumatizing delicate skin can be emphasized by the aging population and the
special needs of geriatric patients. However, this technology also finds utility in
the treatment of neonatal and other chronic wound sufferers. Silicone gel dressings
have been shown to have low peel forces from the skin and have been used effectively
in these applications [65–67]. Shorter-than-desired wear times have limited
the acceptance of silicone gels in the wider market. The resin-containing silicone
gel formulations reported by Gantner and Thomas [51] also have higher adhesion to
skin, which should translate into longer wear times than previously achievable with
traditional silicone gels, while maintaining the characteristic low peel forces from
the skin. Stempel and Leung [54] have combined a cross-linked gel and a silicone
PSA to yield a blend that displays excellent adhesion to steel and ostomy pouches.
The trend toward skin-friendly alternatives has received some attention in the drug
delivery area.
