He had learned 4000 words by the end of last term.按照语法这句话是正确的,
By the end of 18th century ,the city's population ————about two million

为什么答案一定是A was.
我知道by 可以用在过去时态,也可以用在过去完成时态,根据什么确定此时是要填过去还是过去完成时态,如果说按照现实和题意,那么前一句,应该答案选什么?

答：介词by+时间名词构成介词短语作状语时,多用于完成态.
+过去时间时,用过去完成时如：
We had learned 4000 English words by the end of last year.
+将来时间时,用将来完成时态如：
We will have learned 5000 English words by the end of next year.
这两种情况下,主句的谓语动词是一个短暂性的动作（行为）动词,表示一个动作某个时间之前已经发生了.
是的,我们也见到过by+时间名词作状语时,用于一般过去时态的句子:
By the time we reached home,it was quite dark.
我们回到家时,天已经很黑了
这时候,主句的谓语动词是一个表示状态的动词或延续性的动作（行为）动词.
也不是说状态动词（延续性动词）与by+时间短语不能用完成时态.
我们知道,完成态有两种：短暂性用法和延续性用法.前者不能加for+一段时间或since短语或从句,后者必须有这两个表示一段时间的状语句意才完整.
By the end of 18th century ,the city's population ————about two million
（A,was；B,were；C,had been；D,being）
为什么答案一定是A was.
这一题句中的by就是=before,谓语动词是状态动词be,表示在18世纪末之前这个城市的人口情况.
我们可以造出一个用完成时态的例句来：
By the end of this year,we will have lived in this city for 10 years.
By the end of last year,we had been here for 10 years.
